Critical Care Consultant - Neuro Intensive Care, St Georges Hospital

We are looking for an outstanding candidate to join the critical care team working on the

Neuro Intensive Care Unit (NICU) at St George's Hospital. NICU is an 18-bed, specialist tertiary referral unit providing care for adult neurosurgical, neurology and stroke patients.

This is an opportunity for an excellent individual to join the critical care team of a progressive London teaching hospital. This is an exciting time for St George's critical care, with funding in place for a new 20-bed unit to add to our existing estate. There are also developments in research, advanced neuromonitoring and a new 'shock call' service. This provides the perfect opportunity for new team members to develop their own interests and support the growth of the department and the directorate.

Applicants should have FFICM or an equivalent intensive care qualification; subspecialty experience in neuro Intensive care; and suitable experience and qualifications in any proposed second specialty. Applicants must be on the GMC Register or within 6 months of obtaining a Certificate of Completion of Training or Certificate of Eligibility for Specialist Registration (CESR). Main duties of the job

The post is initially contracted for 10 PAs per week. The majority of direct clinical care (DCC) activity will be on the neuro intensive care unit, joining the neuro intensive care consultant on-call rota. The post holder will also contribute DCC activity to the Critical Care Outreach Team, which provides 7-day 08:00-18:00 consultant cover.

Current job planning guidance at St Georges expects consultants to work a minimum of 42 clinical weeks per year. There is flexibility in the number of PA commitments that existing consultants offer to the trust and applicants wishing to work less than 10 PAs are encouraged to apply.

The post holder will be required to participate in the ICU consultant on-call rotas. There are 2 rotas on each unit that run in parallel. The first, so-called 1st on rota is a seven-day duty period running Monday to Monday, including Saturday and Sunday nights. The 2nd on rota provides for second consultant weekday cover and includes that weekday night. 2nd on days are worked slightly differently across the 3 units, either as single shifts or as multiple shifts per week.

The post holder will also be expected to spend a portion of Direct Clinical Care with the Critical Care Outreach service which provides 7-day a week 08:00 to 18:00 consultant cover. There is also the opportunity to be involved with the SW London Adult Critical Care Transfer Service and the ICU Survivorship Service.

Cardiothoracic Intensive Care Unit (CTICU)

CTICU has 21 beds and offers all aspects of level 2 and level 3 critical care to cardiothoracic surgery, cardiology and vascular surgery patients (with the current exception of extracorporeal circulatory support). There are also a number of non-cardiovascular patients admitted to CTICU, including those with severe respiratory failure. The admission profile to the unit has changed recently as a result of COVID with a roughly 40:60 split between elective surgery patients and emergency admissions.

Audit data is submitted to the National Cardiac Benchmarking Collaborative (NCBC) and the Society of Cardiothoracic Surgeons (SCTS), in addition to ICNARC.

General Intensive Care Unit (GICU)

GICU is a 24 bedded unit offering all aspects of level 2 and level 3 critical care (with the current exception of Extracorporeal Circulatory Support) to a wide variety of patients. The workload is currently approximately 100% emergency activity (including major trauma). There is a dedicated, post ICU, follow-up team that provides in-patient review, rehabilitation co-ordination and a weekly, consultant delivered, out-patient clinic.

In addition to clinical areas, GICU has integral offices, a common room and a seminar room on the first floor of St James Wing. The department is supported by several administrative staff, a general manager, a nurse consultant and a large team of nursing and allied professionals. Close links exist between the unit and the medical school.

Research

Research is an integral component of our activity. We have an international reputation both for basic science and clinical trials. We have active links and hold management roles in our regional research network and there is ongoing research activity across all units with dedicated Research nurses and administrative support. We are currently actively recruiting to eleven trials (including three COVID trials), spread across all three units, with another three trials in set-up phase.

NICU has recently become the beneficiary of a substantial research fund, which could allow a successful applicant with an academic interest to establish their own programme of research activity. Candidates with an academic interest are invited to contact Dr Argy Zoumprouli.

The post holders are expected to have up to date Good Clinical Practice training and support the research activities of the unit. The post holders will be supported in any research activities of their own.

A robust governance structure exists across the directorate, with individual and cross-unit M&M and governance processes in place.

Post graduate Education / Education and Clinical Supervision of Trainees

All units benefit from both a junior and a senior tier of junior doctor rotas. In addition, we have Foundation Year 1 posts together with a number of clinical fellow posts.

Clinical teaching is an integral part of the delivery of clinical care and the post holders will be expected to deliver clinical and educational supervision. To fulfil these roles, the post holders will need to complete the necessary training as per the current Trust guidance (see https://www.stgeorges.nhs.uk/education-and-research/education/postgraduate-medical-education/supervision/) They will be expected to develop their educational skills overtime and actively participate in educational appraisal.
