Job summary

An exciting opportunity has arisen within the diabetes team for an enthusiastic Band 7 Diabetes Specialist Nurse to join our friendly team.

The Diabetes unit offer a full range of general and special interest diabetes and endocrinology outpatients clinics housed within the Thomas Addison Unit. The emphasis is very much on multidisciplinary delivery of on-going care for patients with long-term conditions, and increasingly this care will be delivered in partnership with primary care colleagues. The unit also provides diabetic chiropody, retinal screening and dietetic services and also has its own state-of-the-art endocrine investigations unit.

Main duties of the job

The Diabetes Centre at St George's Hospitals is at the forefront of advances in diabetes care and provides a number of specialist services. The team includes the Diabetes Specialist Nurses (DSNs), Diabetes Dieticians, Diabetes Consultants & Diabetes foot team. We provide care & support for our in-patient, outpatient, and we are a very established DAFNE and Insulin Pump Centre. We have a joint Obstetric/Diabetes service, Continuous Glucose Monitoring service, and a busy renal and diabetes foot clinic. The diabetes in-patient service continues to expand in line with the increasing number of people with diabetes. The successful candidate will be required to: To work as an autonomous advanced practitioner and effectively deliver an expert service within the specialist area both inpatient and outpatients.o To manage a caseload of patients/clients &ensure that patients and their families are fully informed and supported by providing clinical and social supporto To lead in the development of evidence based clinical practice in line with current research & guidelines & establish clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patientso Perform the role of educator to student dieticians, medics, nursing staff, dietetic assistant and other AHP's as required
