Assistant Cardiac Rehab Practitioner Band 4 0.65 WTE (24 hours per week)12 month contract (Maternity leave cover)
The Croydon Cardiac Rehabilitation Team is an integrated service caring for patients with Coronary Heart Disease (CHD) within the hospital and across the community. The team delivers education and exercise classes for patients recovering from cardiac events.
This is anexciting opportunityfor a proactive individualwith experience of working with cardiac patients. The post holder will be a self-motivated, resilient and organised individual who is keen to develop their skills in cardiac rehabilitation.

The post holder will be part of the cardiac rehab team delivering high standards of care to patients following a cardiac event according to national guidance. The role will include contributing to the assessment of patients for the exercise classes and assisting with the delivery of the exerciseprogramme. They will work both in the hospital and community settings.
Cardiac Rehab aims to help prevent patients with CHD from being admitted and readmitted to Croydon University Hospital and the post holder will have an active role in the team delivering this aim. The team works both in the hospital and the community in a variety of innovative ways and continues to develop services in order to meet patients' needs. Applicants will need to be willing to take on other roles within the team and be able to be flexible in their approach.
The post holder will also need to have good organisational, administrative and IT skills to support the clinicians with booking clinical appointments and managing clinical databases.
The post holder will be fully supported by the Band 6 and 7 Specialist Respiratory Practitioners and there is ample opportunity for development and training for the right candidate.
