Job summary

A brand new, exciting opportunity has arisen at Croydon Health Services (CHS) for a dedicated and passionate pharmacist to support our new Emergency & Acute clinical services. Our department is looking for someone to join our highly respected Acute Medical team and be part of our ongoing project towards improving patient flow in the hospital and working collaboratively with the MDT team to support medicines optimisation.

The successful applicant will have previous hospital pharmacy experience, having provided clinical services to a variety of clinical environments. You will join a team of pharmacists and technicians working in Acute Medicine to optimise clinical input, support medicines management and improve transfer of care.

Under the guidance of the lead directorate pharmacist, you will be introduced to core directorate activities of reporting and analysis of medicines expenditure, clinical governance, risk management as well as involved in the quality improvement projects within the acute medicine team. You will be responsible for co-developing pharmacy services and providing clinical and professional cover as well as training to junior pharmacists, doctors and nurses.

Through this post, the successful candidate will be able to advance their clinical skills even further, through a variety of clinical cases encountered in our emergency units. We will provide clinical development opportunities such as the post-graduate diploma or independent prescribing if necessary.

Main duties of the job

The post holder will provide a clinical pharmacy service across emergency medicine, acute medicine and our admissions & discharge unit with the aim of optimising pharmaceutical care in the early stages of admission and facilitate flow within the hospital. They will work alongside medicines management technicians, the wider multidisciplinary team and services across the interface to ensure safe and timely transfer of care or admission avoidance. They will collaborate with the Lead Pharmacist-Acute Medicine in developing the service, monitoring metrics and delivering outcomes that will demonstrate the value of early pharmacy input in clinical and operational outcomes. They will identify risks and implement appropriate solutions to mitigating these.

The successful candidate will also support and supervise less experienced pharmacists on rotation.
