Croydon Respiratory Team Lead and Clinical Specialist in COPD (Nurse or Physiotherapist) Band 8a
The Croydon Respiratory team (CRT) is a multidisciplinary service, integrated across Croydon University Hospital and the Community. The service provided by CRT aims to improve outcomes for patients with COPD. Activities include hospital avoidance. In-patient work includes implementing the COPD Discharge bundle and identifying patients appropriate for earlier discharge through a Facilitated Discharge Scheme. Patients are followed up by telephone and face to face reviews in the patient's home and in practitioner-led clinics. CRT also provides the home oxygen service assessment and review (HOS-AR) and Pulmonary Rehabilitation in four locations in the community. CRT has also developed a service in the community to support GP surgeries in diagnosing and managing patients with COPD.

The post holder will provide high quality, evidence based clinical expertise for the service, working to develop the competency of the CRT clinicians, promoting clinical excellence and autonomy. They will be an advanced clinical expert in managing COPD patients in both the chronic and acute phases. They will provide both line management and clinical supervision for less experienced CRT clinicians and have an overview of the clinical day to day service delivery. They will deliver an educational role to both CRT clinicians and other HCPs within both primary and secondary care regarding respiratory disease management.
