Job summary
The Transfer of Care Navigator will aim to improve the flow of patients through Acute Frailty and Elderly Care wards across the site, supporting teams to ensure patients receive timely assessment and discharge from the ward areas.A key function to this role is liaising and communicating with stakeholders to prevent delays in this pathway and deliver timely flow across the site supporting the decongestion of ED and supporting patients getting the right care, at the right time in the right place.
The post holder will be part of the Elderly Care operational team and will ensure early intervention and escalation at the key points in the patient's journey to maintain patient safety, high quality patient care, patient flow and performance against the Trust's standards.
This is a non-clinical role. The Transfer of Care Navigator will understand standard key performance indicators (KPI's), processes and work closely with the clinical and operational teams across the site to enable early escalation of issues within the pathway.
Main duties of the job
- Support the timely flow of patients across the site in line with agreed models of operational models
- Work closely with the Discharge Lounge to ensure efficient utilisation and optimise flow
- Support ward staff to use the Electronic Bed Management System effectively to enable appropriate transfers and timely reporting of vacant beds.
- Link with pathology/ imaging to support ward diagnostic escalations
- Liaise with, and support ward nurse in charge to ensure a strong operational presence along the pathway
- Working closely with the Associate Service Manager for Elderly Care wards and escalate any delays to patient discharge or transfer in a timely manner
- Liaise with patients and their families to inform them of patient discharge and to coordinate any complex transport arrangements in liaison with the nurse in charge and ward clerk
- Work closely with the Discharge Team to aid communication of confirmation of care packages for discharge to the wards and to facilitate other discharge dependent actions delegated by the ward MDTs
- Escalate to the Discharge Team any identified issues with the patient's home environment
- Chase any delays for discharge from wards e.g. pharmacy, transport
- Support flow work across the wider division in times of internal incidents due to bed capacity e.g. Acute wards and Emergency Department may require flow support during times of internal escalation
- Provide daily updates to managers on all definite and query discharges across Frailty wards
