Job summary
As a registered nurse the post holder will provide care to elderly with acute health needs in the ward. The post holder will be accountable and responsible for providing a high standard of patient centred care. To carry out care with direct supervision and support from the team. To communicate effectively within the multidisciplinary team. Contribute towards the effective running of the department including patient safe and timely care.
Main duties of the job

Conduct comprehensive assessments to identify frailty and related health risks in older adults.

Develop, implement, and review individualized care plans tailored to the needs of frail patients.

Coordinate multidisciplinary care involving doctors, therapists, social workers, and caregivers.

Provide education and support to patients and their families on managing frailty and associated conditions.

Monitor patients' progress and adjust care plans as necessary to prevent hospital admissions or complications.

Facilitate access to community resources and services to support patients' well-being.

Maintain accurate and confidential patient records in compliance with healthcare standards.

Promote best practices in frailty management and contribute to continuous service improvement.

Participate in training and professional development related to geriatric care and frailty.
