An exciting opportunity has arisen for a secondment to join a highly motivated nursing team. You will need to have proven clinical skills in respiratory nursing with a special interest in Pleural disease to join a well-developed and supportive multidisciplinary team across both sites at Lewisham and Greenwich NHS Trust (University hospital Lewisham and Queen Elizabeth Woolwich)

We are seeking a Clinical Nurse Specialist to join the Multi-disciplinary team providing comprehensive, state of the art service for patients with pleural disease. The candidate should have prior experience of pleural procedural care in both the acute and outpatient setting. This role will enable you to transfer, apply and develop your clinical knowledge and skills to ensure that the quality of care and the needs of adults with pleural disease are met in accordance to current national and Trust guidelines. The post holder will work alongside the clinical Lead specialist nurse to and provide specialist care within pleural intervention nursing.

The pleural CNS will assist with thoracic ultrasound, diagnostic and therapeutic pleural procedures under the supervision of the medical team and the lead nurse.

Main duties of the job

As a specialist nurse you will review patients within both in-patient and ambulatory settings providing advice and treatment, as well as managing your own independently run clinics. You will be required to follow up and monitor inpatients on a daily ward round to support the ward nursing and medical teams to deliver high quality pleural care. Some of the responsibilities will include after care of patients living with indwelling pleural catheters, and providing care for patient's pre, during and post procedures including medical Thoracoscopy and EBUS.

In line with the Trust's Annual Business Plan you will provide a high quality advanced expert nursing/midwifery practice and cost-effective service to patients requiring routine and emergency care, from outpatients, pre-admission through to discharge and review at home.

The Division aims to provide an environment conducive to the on-going development of all staff and advancements in the quality of the service by ensuring provision of a high standard of training, teaching and clinical care within the acute/community setting.

To provide clinical support carrying out independently or assisting with interventional/investigational procedures in which full training has been undertaken based on clinical practice standards.

The Clinical Nurse Specialist will be an advanced and experienced practitioner within their speciality.

All the following are to be carried out while working in close collaboration with members of the multi-disciplinary team led by a Consultant. To demonstrate clinical expertise within the speciality and work autonomously, managing own allocated clinics independently and seeking guidance/support when necessary.

  • To co-ordinate the specialty care within the Trust/CCG, including outreach clinics within the region.
  • To become an essential member, and point of contact and resource for the multi-disciplinary team and patients and carers.
  • Carries out diagnostic/therapeutic interventions as an independent practitioner, interprets, writes reports and commences further possible treatment on the basis of the findings.
  • Carries out complex therapeutic/investigational procedures under indirect supervision of the Consultant. Ensuring that they are carried out using safe practice and under agreed guidelines/protocols to ensure that the highest possible standard is obtained based on competent clinical practice.
  • To demonstrate the ability to develop new skills, to provide an expert high quality, nurse-led service/midwifery-led service.
  • To provide ongoing follow-up care and support (telephone support / nurse led clinics / outpatient clinics and outreach clinics)
  • To develop the service in a structured interdisciplinary way, initiating on-going change and progression in the expanding role of a Clinical Nurse Specialist.

Pleural Disease (Pleural Effusion, Pnemothorax and plueral mass).

  • To provide advice, support and management of patients with pleural disease referred to the Respiratory medicine and interventional service.
  • To assess adult patients both on admission and within outpatient/ Ambulatory care with pleural disease, providing the necessary information , management, advice and treatment follow up.
  • To undertake thoracic ultrasound for diagnostic and therapeutic purposes requiring analysis, interpretation and knowledge of thoracic ultrasound images in order to define management options.
  • To assist with ultrasound guided pleural aspiration (diagnostic and therapeutic) and chest drain insertion.
  • Arrange discharge of patients managed within respiratory medicine with pleural disease.
  • To support consultant led plural disease clinics.
  • To provide and undertake nurse/midwife led clinics that will deliver direct clinical care, monitoring efficacy and compliance of patients undergoing treatment.
  • To provide autonomous, specialist, self-directed care and treatment to patients within the pleural service. Admitting and discharging patients from own caseload as appropriate.
  • Assess, monitor and evaluate disease activity and drug therapy of patients where necessary by initiating and interpreting clinical data, explaining treatment goals and managing side effects.
  • To undertake and participate in education and support for medical staff with the insertion and management of intercostal chest drains.

Medical Thoracoscopy.

  • To work as part of the multi-disciplinary team delivering this new service. The role ranges across the patient journey from initial assessment, the multi-disciplinary meeting, coordinating with endoscopy procedure delivery, monitoring the service through data , audit and assisting with procedures.
  • To act as a resource within the Trust for the management of pleural diseases.
  • To act as a role model for all staff within Respiratory medicine and to work effectively as part of the respiratory team
  • To ensure effective communication within the multi-disciplinary team to achieve an optimal level of service to respiratory patients and their families.

Endobronchial Ultrasound for central thoracic lymphadenopathy.

  • The role ranges from triaging and identifying patients suitable for this procedure, discussing the patients at the multi-disciplinary meetings , monitor the service through data collection and audit of best practice based on evidence.
  • To educate and assist with the development of medical and nursing staff about the procedure.
  • In accordance with Trust policy this will be achieved within the clinical governance framework.
