Job summary
To lead on the management and support of every aspect of a patient's pathway both internally to the organisation and externally (Independent sector and Mutual Aid NHS hospitals) ensuring the oversight of the effective and timely transfer of patients.
To support the Associate General Manager for the RTT management of a specified surgical service(s) elective recovery improvement plan. The delivery of national waiting times targets is one of the Trust's key priorities, and the use of offsite elective capacity in an efficient way is critical to delivering these critical performance targets.
Main duties of the job
Have a full understanding of both national and local policies relating to RTT management.
Be highly proficient on all trust's patient information systems.
Act as a reliable and effective source of information relating to all offsite providers, their facilities, performance and quality of care.
Keep accurate and detailed records relating to progress along pathways for a large volume of patients.
Take responsibility for ensuring that key milestones in the care of every patient being treated offsite are met in full e.g. consent for offsite care, transfer of notes, pre-assessment and treatment, effective follow-up.
