Job summary
The Renal CNS will deliver expert and timely clinical care for known kidney patients requiring non urgent assessment and intervention particularly those with dialysis vascular access problems (HD and PD). The Renal CNS will jointly manage the Rapid Assessment Unit (RAU) and Renal Day Case Unit on Patience Ward and facilitate smooth patient transitions if a patient needs to be admitted.
The RAU and Day case units are currently open Monday - Friday from 08:00 - 18:00, the successful candidate may be required to work at the weekends in the future.
Main duties of the job
The Renal Assessment Unit is a specialised ambulatory service catering to known Renal patients in need of urgent assessment and diagnosis with the aim to avoid an inpatient admission. The goal is to assess, diagnose, and treat patients within a four hour time frame
The inpatient registrar will provide medical oversight, whilst the CNS will collaborate closely with medical and nursing staff to ensure expert timely and high quality care delivery.
The RAU serves as the central hub for the kidney service, with the CNS overseeing bed availability and managing planning and unplanned admissions and discharges. All external referrals will be directed to the CNS.
The Day case Unit comprises of 5beds. Patients are booked in electively for Renal day procedures including infusions from home or from one of the inpatient wards.
The Renal CNS team will have a role with Renal inpatients with specific strategies to support the clinical complexities of the inpatient cohort i.e. supporting Patience Ward and Richard Bright staff with education and training, Increased knowledge of Peritoneal Dialysis and reviewing complex and long length of stay patients to ensure their Nursing care is optimised.
