The Pulross Intermediate Care Inpatient Therapy team are looking to recruit a highly motivated and enthusiastic Band 3 Therapy Assistant to join our team. Part of Guys and St. Thomas NHS Foundation Trust's Community Rehabilitation and Falls Service, the team provides bed- based therapy intervention for patients from Lambeth and Southwark.
Applicants should have experience of delivering rehabilitation to adults and older persons with complex needs, multiple co-morbidities and falls, with support from qualified therapy staff. The post holder will be required to support service developments, administration and clerical duties, and show flexibility when adapting to and leading change.
The post holder will work alongside a multi-disciplinary team to implement timely and person-centred assessments and rehabilitation. The role includes partnership working across teams within the Rehab and Therapy Services and other agencies within primary and secondary health and social care. Excellent communication skills and an ability to prioritise their own caseload of patients are essential.
The post holder will be required to contribute to the weekend therapy service on the Pulross Intermediate Care Ward.
Main duties of the job
To implement rehabilitation interventions to clients in the intermediate care ward or in their homes. Tasks may include (but are not limited to) teaching prescribed exercise programs, practicing transfers and mobility, assisting and promoting independence in personal care tasks, kitchen practice, undertaking access visits to feedback environmental information to therapists and documenting interventions accurately in notes following trust guidelines.
To work as part of a flexible multidisciplinary team (MDT) and undertake tasks delegated by the Occupational Therapists, Physiotherapists and other members of the team.
To communicate effectively with clients and their carers; particularly clients with impairments which may affect their ability to communicate their needs and/or give informed consent to treatment.
To maintain close working relationships with the multidisciplinary team and notify the appropriate team member immediately of any changes in the health of clients to ensure their safety and wellbeing at all times. To report any changes in client's progress to allow for intervention to be adjusted by a Therapist.
About us
The Inpatient Intermediate Care Therapy Team is based on the Pulross Rehab Ward at the Pulross Centre, Brixton. We provide bed-based rehabilitation including physiotherapy and occupational therapy to adults and older people with illness, injury or disability helping them to be more independent in their daily activities prior to returning home. The clients have a wide range of functional difficulties, long-term conditions and disabilities and many have multiple pathologies and complex medical conditions. The rehabilitation team consists of doctors, nurses, physiotherapists, speech and language therapists, occupational therapists, pharmacist, social worker and dietician. It serves patients from Lambeth and Southwark
Aside from the Pulross Rehab Ward, clients may be seen in their own homes and in health and community centres within Lambeth and Southwark. The service leads the provision of Falls prevention and management of clients within both boroughs providing individual and group exercise programmes.
To support the service's commitment to meeting client needs and delivering excellence, it is essential the post holder be able to work their hours flexibly and contribute fully to both existing and new rotas. This may include 7 day working and non- traditional shift patterns.
