Job summary

We are delighted to invite applications for the position of Specialist Rotational Clinical Pharmacist within the Immunotherapy Clinical Pharmacy Team. Due to growth in activity, primarily in Dermatology and Allergy we have secured an additional specialist clinical pharmacist post. The successful candidate will rotate through our adult outpatient based clinical specialties alongside our existing rotational band 7 pharmacists. These specialist clinical rotations allow individual pharmacists to gain the knowledge and skills necessary to develop into a highly specialist clinical pharmacist in secondary/tertiary outpatient based long term conditions. Whilst each rotation covers different clinical specialties, the core skills involve the management of biologic and non-biologic disease modifying agents administered on site via one of our outpatient infusion/day units or direct to patients at home.

The one year rotations are:

1) Neurosciences, Chronic Pain and Ophthalmology

2) Dermatology and Allergy

3) Rheumatology and Metabolic Bone

Main duties of the job

This clinical rotation builds upon the skills, knowledge and experience gained at core clinical rotation level and encourages you to take an active part in experiential learning. The successful candidate will work within a team often Highly Specialist Pharmacists and Technicians. Your clinical practice will be to deliver high quality clinical care to a cohort of medically complex patients that require their long term conditions to be managed in secondary / tertiary care.

You will gain experience in the use of high cost/high risk medicines, clinical commissioning, local clinical pathways, outpatient clinics and actively participate in multidisciplinary team meetings. You will also deliver and track medicines optimisation projects within the clinical specialities under the supervision of the Highly Specialist Pharmacists.

To provide a clinical pharmacy service in accordance with the priorities set by the clinical team leader for each rotation in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

  • To identify and manage risks in relation to medicines use according to policy/protocol.
  • To delegate duties / responsibilities appropriately to junior staff.
  • To evaluate junior staff performance against identified criteria using departmental/Trust PDR tools and General Level Competence Framework.

  • To provide evaluated pharmaceutical and financial information in accordance with the needs of the directorate.
  • To identify changes in prescribing practices that may impact on the drugs budget.

4.4 Evaluation of Service

To monitor clinical pharmacy in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

  • To document the workload of pharmacy services in line with departmental and corporate objectives.
  • To document the quality of pharmacy services in line with departmental and corporate objectives.
  • To undertake clinical audit as part of the multi-disciplinary audit process.
  • To undertake assessments / evaluations as required and be able to apply the key principles of giving feedback.
