Job summary

The Homeless Service is an excellent award-winning service (MBE award, two-time Winner of Lammy Award, Regional Winner of Health and Social Care Award for Improving Health and Reducing Inequalities, Winner of GSTT Care Award, Winner of Ab Fab Award) and friendly team delivering services to those experiencing homelessness. The post holder will be predominately working in non-NHS sites such as hostels, day centres and street nursing when required and will be expected to travel to the Gracefield Gardens office when needed. We offer flexible working opportunities.

We are currently recruiting to a Band 6 Community Nurse Specialist (Adult) secondment or fixed term post, with potential for development. This post is to provide exceptional high-quality nursing care for those who are risk or homelessness or rough sleeping in Lambeth, Lewisham and Southwark. This is an exciting opportunity to work with a marginalized cohort with complex needs, working in a multidisciplinary team with experts who are leaders in the specialty of Inclusion Health. The new team member will be supported by senior staff, who have many years of experience and knowledge. This exciting role, will ensure that vulnerable adults are able to access the healthcare and support they need. Prior to applying for a secondment, check with your line manager.

Main duties of the job

The Health Inclusion Team works across Lambeth, Southwark, and Lewisham to provide a high quality comprehensive specialist nurse service for clients who have difficulty accessing primary care services e.g. refugees and asylum seekers, homeless clients, and clients with addictions and high users of A&E. The Health Inclusion Team aims to assist these clients to access mainstream health services.

Key services provided are:

  • Assistance with GP registration,
  • Health screening (i.e. TB, blood borne virus screening, sexual health screening, baseline routine blood tests)
  • Comprehensive health assessment
  • Minor illness / injury management
  • Chronic disease management
  • Harm reduction work
  • Wound care
  • Case management of complex clients
  • Referral to other services as appropriate
