Main duties of the job

Clinical posts Unique out-of-programme FY3+ training rotations in older adults' medicine and frailty pathways from hospital to the community, rotate through three placements; older persons' unit wards, acute ambulatory-OPU in the emergency floor, GSTT hospital @ Home in the community, and one speciality outpatient clinic per week. Also 6 month posts in hospital@home applying medical skills to acute care in the community combined with technology.

Research and Teaching posts A unique year developing research skills, applying for grants, setting up and recruiting to a range of frailty and cardiac disease research projects, publishing results. Delivering and improving undergraduate medical school modules. No previous research experience required- just enthusiasm and a willingness to learn.

All ICFs participate in quality improvement programmes and teach undergraduates.

There is a monthly ICF training programme and a wealth of departmental and directorate educational meetings.

Some roles participate in an on-call rota, currently no role is required to do night shifts. Where roles match IMT requirements , IMT equivalence will be supported.

The posts provide excellent Geriatric & General Medicine training and opportunities to develop research and teaching skills. Ideal for those seeking to determine whether to follow a career in General Medicine, Acute Medicine, Geriatric Medicine, General Practice or Academic Medicine.

Main place of employment:

This post will be based within the community. The Hospital@Home bases are in Elephant & Castle and Tulse Hill. North Southwark Integrated Neighbourhood Hub is in Bermondsey. Care homes are across Lambeth and Southwark but the post holder will be linked to 1-2 homes. Home visiting in Lambeth and Southwark is expected in this post. Post holders will also have a base at St Thomas' Hospital and will join in with departmental educational activity.

Job responsibilities

Clinical

There are 2 FTE posts which may be delivered in greater numbers of posts with LTFT trainees. Post holders will have opportunity for placements in GSTT Hospital@Home service, North Southwark Integrated Neighbourhood hub, care home support work as well as other opportunities in community delivered services. There will be opportunity to tailor the post where possible to meet individual post holders interests. Part-time applicants are welcomed, whether this be trainees wishing to mix in & out of programme activities or whether due to LTFT work. We are supportive of working with applicants to bespoke working patterns to individual needs where possible.

  • The Hospital@Home service: provides acute care and treatment at home as part of a large multidisciplinary team. This will involve conducting home visits for initial assessments and ongoing care, leading multidisciplinary team meetings, conducting hospital in-reach and engaging with education, governance and quality improvement in this unique setting. Supervision will be via the consultant geriatrician and GP on duty in the service.
  • The North Southwark Locality hub will be developing a new innovative frailty support model for community-dwelling older people living with frailty and advanced/multiple comorbidities. This will involve working with GPs, pharmacists, experienced nurses in frailty and dementia and the wider MDT in the hubs. Supervision will be via GPs and consultant geriatricians.
  • There are no night shifts in this post and no working beyond 8pm. Some community based weekend work is included in this post

Training of junior staff:

The post holder will take responsibility for the training and direction of junior staff allocated to him/her with their supervising consultant.

Teaching:

The post holder will be expected to contribute as appropriate in the teaching of undergraduate and postgraduate students.

The post holder has access to the full range of training opportunities in the Trust including through the School of Improvement.

Clinical Governance:

All medical and dental staff are expected to take part in clinical governance activity, including clinical audit, clinical guideline and protocol development and clinical risk management. They will be expected to produce evidence of their contribution in these areas and their audit of their own clinical work as part of their appraisal.
