Job summary

This post is designed for an ST4 - 7 level trainee in Emergency Medicine to:

  • Develop / expand their skills in Frailty Medicine in a specialist clinical and academic centre.
  • Undertake research in the field of Clinical Frailty

The post will count for 6 months training time towards a CCT in Emergency Medicine provided total training time has not been exceeded.

The job will allow the successful candidate to undertake focussed training in a mix of environments related to Ageing and Health, in addition to gaining Emergency Medicine (EM) experience in a busy London teaching hospital. The successful candidate will spend time in Emergency Medicine, the Hospital @Home team and the Acute Older Persons Unit (AOPU).

The successful candidate will have the opportunity to work in delivering acute care, Comprehensive Geriatric Assessment (CGA) as well as end of life care support for older people in the community setting. The post offers the opportunity to work with our established Hospital@Home team. This will involve seeing in the pre-hospital setting, taking referrals from London Ambulance Service (LAS), assessing patients at home and forming management plans to help avoid unnecessary admissions. The post holder will also join the Hospital@Home MDM delivering Comprehensive Geriatric Assessment in people's homes.

Main duties of the job

Emergency Department: When on duty in the Emergency Department, the post-holder will be expected to work with the Consultants in direct patient management and supervising the work of the junior medical staff in both the adult and paediatric areas. He/she will also lead treatment of patients in the resuscitation area. The Registrar is expected to advise the junior doctors and Emergency Nurse Practitioners on patient management and to authorise referrals to speciality teams. He/she will be in charge of the department overnight (with another ST4+, and a Consultant on-call). The Registrar will assist the Consultants in the clinical and administrative management of the Emergency Department.

Ageing Medicine and Health role:

  • Hospital@Home. When on duty with the Hospital@Home team the post-holder will work as part of the multidisciplinary team assessing patients referred from the ambulance service and providing an alternative to hospital admission. They will conduct home visits and assessments, perform point of care testing and develop patient centred management plans. The post holder will be involved in the ongoing home based care of patients through follow up home visits and MDMs. Supervision will be via the consultant geriatrician and GP on duty in the service.
  • AOPU (Acute Older Persons Unit) The post holder will work as part of the geriatrician lead AOPU (acute frailty service) providing front door comprehensive geriatric assessment of frail patients.

Opportunities for Training and Research during the OOPTE

For the year 2023-24 St Thomas Hospital has chosen to participate in CQUIN 05, Identification and Response to Frailty in Emergency Departments. Part of the role will be working across the Emergency and Geriatrics departments to ensure that targets are being met and that the pathways are optimised to improve the care that is being delivered to the Frail population visiting ED. The Ageing and Health Department at GSTT has a wide research portfolio and there will be opportunities available to the post holder to engage in research and quality improvement projects aligned with their interests.

Management

The successful candidate will be encouraged to participate in a number of management activities with the specific aim of developing key management skills which are needed in future Consultant Posts. A management mentor will support the successful candidate with key management objectives. Support for future consultant applications including interview practice will be provided

Educational Program

The successful candidate will be given the opportunity to teach on/attend appropriate conferences and specialist courses including ATLS, ALS, APLS and simulation training within a set study leave allocation. The post holder will be expected to attend and present at a variety of educational meetings:

  • Weekly departmental middle grade teaching programme (alternating Frailty and Emergency Medicine)
  • Weekly M&M meeting
  • FY2, medical student and nurse teaching
  • Teaching on Silver Trauma courses
  • Hospital Grand Rounds and Afternoon Report
  • Dedicated Geriatric medicine registrar teaching
  • National and International Meetings as indicated

The post holder has access to the full range of training opportunities in the Trust including through the School of Improvement.

Timetable and Salary

Emergency Department: The Emergency Department operates a full shift system, which is fully compliant with the European Working Time Directive. The post holder will participate in the night and weekend rota. All shifts are either 9 or 10 hours in length. The rota uses self rostering to facilitate a positive work-life balance. The post holder will do 60% of the total night rota and 75% of the weekend rota. Pay supplement at band 1A applies to this post. Salary is in accordance with the national salary scale. There is no on-call commitment to the Emergency Department but rotas may need to be amended to cover unexpected sickness.

Ageing Medicine and Health: There are no night shifts in this part of the job and no working beyond 8pm. Some community based weekend work is included in this post. Please see below for an example fortnight timetable of work.

Training of junior staff:

The post holder will take responsibility for the training and direction of junior staff allocated to him/her under aegis of the training plan that that post holder has agreed with their supervising consultant.

Teaching:

The post holder will be expected to contribute as appropriate in the teaching of undergraduate and postgraduate students.

Clinical Governance:

All medical and dental staff are expected to take part in clinical governance activity, including clinical audit, clinical guideline and protocol development and clinical risk management. They will be expected to produce evidence of their contribution in these areas and their audit of their own clinical work as part of their appraisal.

Mutual Obligation to Monitor Hours:
