Job summary

A new vacancy has arisen for a Clinical nurse specialist in mental health to join an established mental health liaison service within the Heart, Lung and Critical Care (HLCC) clinical group of Guy's and St Thomas' NHS Foundation Trust (GSTT). The current post has arisen to enhance the team's capacity to respond to the acute mental health needs of inpatients at the hospitals.

This role will primarily be based at the Royal Brompton Hospital (RBH) with some cover on the Harefield Hospital (HH) site as needed. There is a shuttle bus provided between the RBH and HH sites.

The setting the post holder will work in is a specialist heart and lung hospital which is well known for its innovation, research and forward thinking approach.

Main duties of the job

  • To work collaboratively with staff on the RBH and HH sites to provide a responsive mental health assessment, advice and treatment service
  • To recommend and set up discharge plans and enable users to access appropriate on-going care or treatment from community services
  • To make clinical note entries in the electronic medical records that are comprehensive, clear and concise in line with GSTT guidance
  • To develop and provide a rolling education programme to RBH and HH staff
  • To promote mental health awareness and develop links with other non-mental health services within the RBH and HH sites
  • To work collaboratively with carers and families to provide support, advice and information
  • To provide a proactive approach with patients that find it difficult to engage
  • To complete regular audits

About us

The post holder will be part of a team that consists of:

  • 1.0 WTE Lead Nurse for Mental Health within HLCC for the RBH and HH sites (cross-site)
  • 0.8 WTE Consultant Liaison Psychiatrist (cross-site)
  • 2.0 WTE Clinical Nurse Specialist in Mental Health at RBH (with cover at HH as required) - of which 1.0 WTE is this advertised post
  • 1.0 WTE Clinical Nurse Specialist in Mental Health at HH (with cover at RBH as required)
  • 0.2 WTE Administrator (based at HH)

The team works widely across all directorates at RBH and HH. Clinical supervision is provided by the Lead Nurse in Mental Health and/or the Consultant psychiatrist.

Communication

  • The post holder will be operationally accountable to the Lead Nurse in Mental Health within HLCC for the RBH and HH sites and the Consultant Liaison Psychiatrist for day-to-day operational and managerial issues.
  • To provide advice, support, and consultation regarding the psychological and mental health management of patients and their carers, to healthcare professionals on the postholders primary site, and at the other site during times of cover.
  • The post-holder will prioritise and organise their work effectively and efficiently, including the management of new referrals, an on-going case load and discharges.
  • To contribute as a member of a multi-disciplinary team to the overall mental health, psychological and medical care of patients of RBH and HH.
  • To work collaboratively with all members of multidisciplinary care teams at RBH and HH, providing support and advice where appropriate and attending MDT meetings.
  • To demonstrate excellent communication skills and to be able to promote cohesive relationships, look objectively at problems, and problem solve effectively.
  • To complete comprehensive mental state examinations including risk assessment and risk management plans in collaboration with relevant healthcare professionals.
  • To be responsible for recommending evidence based psychological and psychiatric interventions and planning and reviewing all interventions and care plans.

  • To provide mental health care appropriate to the clients physical health needs and make appropriate referrals when necessary to the patients local primary care services and/or psychiatric/psychology services.

  • To attend mandatory and statutory training as required.
  • To be a source of knowledge and advice for staff at RBH and HH on current mental health legislation (Mental Health Act, Mental Capacity Act, Deprivation of Liberties, etc) and developments in mental health.
  • To help identify specific mental health related training needs for RBH and HH staff and contribute to training programmes required to meet these needs.
  • To contribute to training workshops for RBH and HH staff on evidence-based interventions for the management of psychiatric emergencies.
  • Any other teaching commitments that may be required or requested by the Lead Nurse or Therapy/Service Leads at RBH and HH.

Continuing Professional Development

  • The post-holder will be expected to undertake an annual appraisal and maintain an up-to-date Personal Development Plan (PDP).
  • The post-holder will be expected to attend regular supervision and be able to critically reflect on their work responsibilities and performance. The postholder may also be expected to provide reflective supervision for other designated staff e.g. palliative care.
  • The post-holder will be expected to maintain an expert level of competence by identifying training needs and attending relevant training courses and conferences.
