This position will support patients and health care professionals with delivery and implementation of the Vital 5 Screening and Make Every Contact Count (MECC). The Vital 5 project advocates for people, communities and organisations to make improvements in/to modifiable risk factors for ill-health. These key drivers have a major impact on health at an individual and population level and are known as The Vital 5: blood pressure, tobacco, alcohol, healthy weight, and mental health.

MECC supports the opportunistic delivery of consistent, simple health messages that signpost residents to services and information making the most of everyday opportunities to empower people to improve health and well-being. MECC is a vehicle supporting person-centred conversations, empowering people to make healthier choices. It is a concept which aims to improve lifestyles and reduce health inequalities.

A MECC approach to Vital 5 aims to prevent ill health, promote good health, and improve detection, management and optimised treatment of existing conditions.

Main duties of the job

Main duties of the job

  • Complete assigned tasks as part of clinical team in support of registered nurses and to undertake some aspects of training and care provision without direct supervision.
  • Undertake vital signs
  • Work in collaboration with the MECC Practitioner and MECC Matron to support the delivery and embedding of the 'Making Every Contact Count' and Vital 5 programme across GSTT Outpatient Services.
  • Support the delivery of Vital 5 screening
  • Support other HCAs to deliver Vital 5 / MECC
  • Attend and contribute to team meetings and events as required
  • Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role
  • Ability to work to and prioritise deadlines
