Job summary

This is an exciting opportunity for a senior registrar / fellow to join our combined ECMO & teaching fellowship at the Royal Brompton Hospital (RBH) and Harefield hospitals (HH). The posts are for 1 year, with 6 months at each site. The expected start date is February 2024.

The post would suit doctors (ST4+ equivalent) within a critical care training programme. As well as being involved with ECMO the fellowship will aim to develop leadership, service delivery and management skills. A fundamental part of this fellowship will require them to be involved in assisting in the delivery of the registrar training programme in critical care that is delivered across both sites.The fellow would be expected to participate on the on call rota.

Main duties of the job

CLINICAL RESPONSIBILITIES

  • To be part of the critical care team based across the Royal Brompton and Harefield critical care units as required.
  • To participate in medical audit.
  • To participate in the education and supervision of medical trainees and other staff.
  • Critical care echocardiography (TTE and TOE); rom FEEL/FICE, BSE Level 1-2, CUSIC certification to BSE/EACVI accreditation and beyond
  • ECMO (VV and VA) and retrieval
  • Additional opportunities maybe possible after discussion (respiratory medicine, pulmonary hypertension, anaesthesia, transfer medicine, GUCH, ICU follow up, management & leadership, education)

About us

RBHis a specialist cardiothoracic center withapproximately 2,081 staff, 296 beds,2 critical care areas, 6 operating theatres, 5 catheter laboratories, extensive imaging facilities.The Hospital benefits from a joint cardiac and respiratory clinical research facility and has strong links both with King's College London/ King's Health Partners and the National Heart and Lung Institute at Imperial College.

RBH critical care supports a variety of cardiological and respiratory diseases and post procedural/operative cardiovascular and thoracic cases across two critical care areas (AICU, Elizabeth and ICU). Tertiary and quaternary services include ECMO (cardiac and respiratory), Grown Up Congenital Heart (GUCH) disease, pulmonary hypertension, cystic fibrosis, and asthma, and minimally invasive cardiological interventions. There is an active teaching programme covering general and specialist critical care. This includes journal club, lectures with internal and external speakers.

Royal Brompton & Harefield Hospitals have merged with Guy's and St Thomas' NHS trust with the formation of a Heart, Lung and Critical Care Clinical Group. During this period of exciting change, the organisation is developing a shared learning programme for governance, guidelines, education, research, and service innovation.

RBH is a commissioned VV ECMO center and is also part of the London Shock service and manages patients who are in cardiogenic shock and require VA ECMO and temporary Left ventricular assist devices such as Impellas. There will be opportunity to learn about the management of patients on ECMO and also participate in inter and intra hospital transfers of patients on ECMO. Furthermore, we will provide ECMO courses for the fellows and opportunity to attend international conferences. The RBH is fully invested in research and innovation and there are a wide numbers of research opportunities with expectation of presentation at conferences and publications. We will assist with development of any specialist interests that fellows may have such as echocardiography training.

Harefield Hospital has 39 critical care beds, five theatres, four catheter labs supporting a busy primary PCI service, heart and lung transplantation as well as complex cardiothoracic surgery. Being a heart and lung transplantcenter,Harefieldspecialisesin all forms of mechanical circulatory support. At HH the post holder would be expected to contribute to the cardiogenic shock andeCPRprogram, and gain experience of VA ECMO,impella, ventricular assist devices as well as post operative management of heart and lung transplant recipients. They would also support the mechanical life support service, which organizes monthly courses involving simulation based teaching of all forms of MCS, with potential for research opportunities in this area.

The units have consultant cover 24/7, onsite day times Monday to Sunday, and additional consultant cover is provided for ECMO retrieval.

There is an active teaching programme covering general and specialist critical care, as well as targeted teaching for relevant postgraduate examinations, either locally or in collaboration with neighbouring institutions.

The unit teaches and trains undergraduate students from Imperial University and has numerous honorary visiting fellows. The postholder would be expected to support these teaching activities.
