Job summary
We are looking for two experienced, enthusiastic pharmacists to lead pharmaceutical care to our gastroenterology and surgical patients in both the inpatient and outpatient setting at Guy's and St Thomas'. (You will find 2 job descriptions, one for the inpatient and another one for the outpatient) If you are a proactive, problem solving medicines expert who enjoys training doctors and nurses and can lead a team of ward based pharmacists and technicians to provide the best pharmaceutical care, you're just the person we need!
You will be able to plan and communicate medicines solutions for patients with complex co-morbidities in acute post-operative complications, teaching others to prioritise, plan and deliver expert pharmaceutical care. You will work closely with colleagues to identify medicines support needed across surgical specialities, adapting to pathway changes as the Trust creates novel ways to increase our surgical activity.
Main duties of the job
Highly Specialist Pharmacist: GI Medicine and Surgery
Guy's and St Thomas' is a national referral centre for complex colorectal disease and the South East regional unit for upper gastro-oesophageal surgery. Your patients might include anything from an emergency appendicitis or cholecystitis, to hepatic encephalopathy or an acute episode of ulcerative colitis. It will include specialist surgery for complex fistulae, IBD, or advanced malignant disease, with changing patient acuity and routes of administration where the MDT turn to you for answers. This role will work closely with the gastroenterology outpatient pharmacy team and act as a liaison to the ward service, you will be trained to take on responsibilities such as supporting the IBD infusion unit and immunomodulator management where necessary.
Highly Specialist Pharmacist: Gastroenterology and Medicines Optimisation
Guy's and St Thomas' is a specialist IBD centre, the successful candidate will be integrated within the MDT to provide expert pharmaceutical care to our patients. You will conduct pharmacy led clinics, high cost drug/advanced therapy management, deliver on medicines optimisation projects and contribute to the delivery and development of the gastroenterology outpatient pharmacy service. This is a dynamic role where you will be required to keep up with the ever-changing landscape of IBD treatment and will work across many different teams to ensure our patients have access to new medicines coming to market.
About us
Working with specialist colleagues in orthopaedics, plastics, VTE prevention and anticoagulation safety, you'll jointly lead our team in delivering medicines expertise across elective and emergency inpatient pathways in gastrointestinal (GI) surgery and medicine. This includes gastroenterology inpatients, upper GI, colorectal, general and endocrine surgery, including troubleshooting acute pain issues. We work as specialists but support one another across our whole team service to provide the best proactive care and advice.
