Job summary
The Host Defense (HD) service cares for patients with bronchiectasis, fungal respiratory infections and non-tuberculosis mycobacterium.
Our service provides both inpatient and outpatient care, including a busy and thriving OPAT IV service. Twice weekly Host Defense clinics and ward rounds, monthly fungal MDT.
Patients are reviewed in virtual clinics as well as the traditional face to face clinics.
The HD CNS team provide a reactive service, answering patient queries ad-hoc via telephone and email.

Main duties of the job
- To be a highly specialised nurse working with a specified group of patients, taking responsibility for supporting their on-going clinical management.
- Acting as a carer and advocate for those patients, giving expert support and advice based on best practice with close communication with colleagues in each specialist area.
- To be a primary point of contact for individuals with lung infection, non-CF bronchiectasis and primary immune deficiency (PID).
- To be the lead and line manager role for the band 6 Infection and immunology nurse and mentor support for the B7 Primary Cilia Dyskinesia as appropriate.
- To be a resource for multidisciplinary colleagues.

- Provide a service for all patients, including direct care and acting as a specialist resource when liaising with and facilitating the best use of all available services.
- Assess, plan, implement and evaluate programmes of care.
- Facilitate and monitor the management of care for inpatients and plan discharge in conjunction with the ward.
- Act as an expert resource for all patients, giving advice, education and support that is appropriately planned to meet each individual's needs empowering them to manage their treatment regimes, to recognise any deterioration or acute episodes requiring immediate specialist attention and how to access the necessary support.
- Organise and coordinate the PID outpatient clinics, assisting the consultant immunologist in the clinical management.
- Triage patient queries providing antibiotic advice and organise sputum collection. Liaise with GPs to request oral antibiotics.
- Provide advice to local hospitals regarding antibiotic choice for intravenous therapy.
- Participate in Advance Care Planning and end of life care planning of both in-patients and outpatients, liaising with local services as necessary and working with the palliative care team to ensure that the patients and familys needs are met throughout.
- Monitor all patients receiving intravenous therapy at home arranging and checking aminoglycoside levels, arranging follow-up and line removal, making contact throughout the course for support and to answer queries.
