Job summary

We are looking for a vibrant and dynamic person to join our award winning, pioneering and integrated team in this exciting role.

The post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care in their usual place of residence. This includes a 2 hour response for patients referred to the service as part of our Urgent Care Response pathways which responds to patients who have health, therapy and social care needs.

The successful candidate will demonstrate a high level of expertise and skill in their work providing advice, education and support to health care professionals and patients. They will actively undertake clinical audits and team management, working to set and maintain high standards both clinically and non-clinically.

The successful candidate will be able to assess, treat and plan care for acutely unwell patients at home (including care homes), to support advanced discharge from hospital and prevent unnecessary admissions. To effectively liaise with community partners to facilitate admission, transfer of care and discharge which is patient and customer focused and reflects best practice.

This role will be based within the community visiting patients in their own homes so the ability to drive and hold a valid driver's licence will be invaluable.

Main duties of the job

This role involves direct patient care within the community for acutely unwell adults.

You will be able to independently manage a group of patients whilst working as part of the multidisciplinary team, delivering individualised and personalised direct patient care. Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care and requesting of agreed diagnostic procedures/clinical investigations. You will be responsible for the correct administration of prescribed medication including intravenous therapy. Be responsible for assessing and recognising emergencies in the speciality. You will also be able to interpret information and take appropriate action and lead others to do the same.

You will be expected to act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. To be pivotal in encouraging and managing appropriate referrals, attending ward rounds, MDT and other meetings. Ensuring that patients receive high quality clinical care and a good patient experience, having regard for their social circumstances, customs, religious beliefs and doctrines. Supporting and enabling patients and carers to make informed decisions relating to their treatment and management.
