Job summary

Two exciting opportunities have arisen for suitably experienced nurses within the Neighbourhood Nursing Team as B6 Triage Nurses on a 12 months fix-term contract, offering an excellent opportunity to grow and develop in leadership, management and clinical assessment skills as well as trying out a new role at this level. This is a dual role where half of the post goes into clinical practice and the other half with referral triaging, working with the Single point of access to improve the quality of triage within the service.

You must demonstrate excellent communication skills and team working. Through clinical case management you will care for patients with long-term conditions, those at risk, at end of life, and those with supported self-care programmes. You will also provide care for patients with complex and challenging needs.

Training and opportunities will be provided for you to develop your skills and competencies, in addition to clinical supervisions and appraisals.

The hours will primarily be 9am to 5pm, Monday to Friday when the majority of referrals come into the service but maybe subject to change to include weekends working depending on the needs of the service. The post will be based in the Powerhouse Building in Kennington and will require on site working.

Main duties of the job

This is a newly designed post to provide clinical and professional leadership to all aspects of referral management working within the Single Point of Access team (SPA) and with the Neighbourhood Nursing (NN) teams. It is an opportunity to demonstrate the impact of effective referral management within a busy community nursing service. It will involve dealing with a daily stream of referrals, with effective triaging for eligible patients.

The postholders will develop methods of establishing the housebound nature of patients and produce a best practice triage guide for use by nurses within the service undertaking triage. They will also provide training to nursing staff on referral triaging. Additionally, the post holder will undertake clinical visits to housebound patients to provide nursing care.

  • To act as the first point of contact for new referrals into the Neighbourhood nursing service.
  • To carry out telephone triage of referrals via supportive listening to determine priorities of need, risk and coordinate onward referral to the most appropriate community service.
  • To undertake clinical visits to housebound patients with complex care needs to plan and deliver nursing care.
  • To advise and signpost referrers to where appropriate services can be obtained for patients and clients.
