Job summary
The role is to ensure seamless and effective care of the patient by working between the medical and ward nursing teams, troubleshooting problems and escalating to the senior surgical team when required.
You will be expected to act within an highly specialized role model and leader for all members of staff, and play a major role in the continuous development of the services within the Trust and as a potential member of a range of Trust quality/process improvement project groups.
The role will include ward cover and management of urgent referrals for cardiac surgery (pre and post-operatively).

Main duties of the job
- To deliver evidence based expert nursing care with a high degree of professional autonomy demonstrating the ability to make sound decisions based on theoretical knowledge and experience.
- Communicating with Patients in such a way as to empower them to make informed decisions regarding their management and actively promote their health and wellbeing.
- Daily ward round with Consultant of the week, MDT including the ward Specialist Registrar (SpR), Pharmacist and the nurse in charge. Plan and manage complex programmes of care effectively, through working in partnership with Patients and other professions.
- Daily review of patients' medication, assessing the effectiveness of current therapy and independent prescribing as required. Also to prescribe TTOs for all patients with a discharge plan, in order to facilitate a quick discharge from Hospital. All of these is done in with the Pharmacist on the ward, as a team work, to enable cross-checking and reduce the risks of incidents (e.g. wrong prescription, wrong doses/duration of treatment, and others).
- Admission clerking including prescribing (advanced physical assessment), in conjunction with the Pharmacist on the ward (cross-checking with medication reconciliation).
- To be involved in Audit and evaluate service. Identify any gaps in the service and how these can be filled.
- Providing professional and clinical leadership and a resource for education.
