Job summary

This 6 to 12-month post offers candidates the opportunity to gain experience in Intensive Care Medicine (ICM) within a busy District General Hospital. Accreditation as the University Department of Critical Care Medicine was awarded in 2022 and in parallel with exceptional clinical experience, this post will also offer the exciting opportunity for dedicated clinical research time.

This job would be ideally suited to individuals who are committed to or strongly considering a career in ICM or a related acute speciality, and who are hoping to develop an ongoing interest in clinical research. It would also be appropriate for Faculty of Intensive Care Medicine (FICM) trainees seeking their special skills Stage 2 training. Some experience of ICM is strongly desirable.

Main duties of the job

Objectives of the fellowship would include:

  • Participate in provision of ICM within the department at a resident doctor level
  • Undertake the NIHR Associate Principal Investigator programme
  • Support recruitment to and data collection for ongoing studies within the ICU
  • Attend Urgent Care research committee meetings
  • Be involved in the management and co-ordination of studies within the ICU

About us

Our ICU has recently been rated 'Outstanding' for patient care by the CQC and the Trust overall has a 'Good' rating. The unit currently admits nearly 1000 level 2 and level 3 patients per year into 23 beds. The case mix is predominantly medical with trauma and surgical patients making up about 20%. Around 90% of our admissions are emergencies with the remainder being high risk surgical cases including, colorectal & urological procedures. There are about 60 paediatric admissions and stabilisations a year, the vast majority of which are retrieved via the Southampton and Oxford Retrieval Team (SORT), but some remain for the initial stages of their stabilisation and treatment. The Trust is a tertiary referral cancer centre and includes all specialties except neurosurgery, transplant, vascular and cardiothoracic surgery. The trust has a primary coronary angioplasty service with the fastest door to needle times in the country as well as the fastest door to needle times for stroke thrombolysis. There is an excellent interventional radiology department with 24-hour service provision and frequent close working with ICU. We also receive excellent support from our microbiology department with daily ward rounds on the ICU.

Job responsibilities

We are a recognised regional centre for training in ultrasound and echocardiography. We have close links with the cardiology department and Dr Elkington, consultant cardiologist, who is one of our local BSE supervisors. We also have close links with Dr Andrew Walden in acute medicine, who was involved in establishing point of care ultrasound within the hospital and also its national training process. Within the Department of Critical Care Medicine multiple consultants hold point of care ultrasounds skills ranging between full BSE accreditation, FUSIC heart, FUSIC lung and CACTUS (childrens point of care ultrasound). There are many opportunities for mentorship, education and participation in developing these skill sets. Both FAMUS, FUSIC and CACTUS courses are regularly run and are attended by delegates from around the country with excellent feedback.

The department has a firm education and teaching programme with weekly, dedicated, bleep-free sessions for resident doctors to attend. These are overseen by multiple senior educators who also hold positions and undertake regular teaching within the University of Reading. We have high fidelity simulation facilities within the hospital and also have a dedicated facility within the ICU to provide in-situ simulations for MDT training in multiple types of clinical, human factors and patient safety aspects of ICM.
