Job summary

Band 6 - Renal Social Worker

We are looking for a Renal Social Worker who is able:

  • To provide a social work service to clients under the care of the renal department.
  • To provide a full psychosocial assessment of patients and continuing psychosocial support.
  • To liaise with other health and social care professionals both within the Trust and the Community.
  • To work as a member of the renal multidisciplinary team (MDT) and to provide appropriate social work input to that team.

Main duties of the job

  • Find solutions to problems ie Financial & benefit assessments.
  • Psycho- social support.
  • Multidisciplinary team Meyerbeer of the Renal team.
  • Helping protect vulnerable people renal patients from harm or abuse.
  • Supporting renal patients to live independently where appropriate.

  • Certificate of qualification in social work (CQSW) or Diploma in Social Work Appropriate Degree 3 years post qualifying experience Current registration with the Health & Care professional council (HCPC)
  • Current registration with the Health & Care professional council (HCPC)
  • 3 years post qualifying experience

Desirable

  • Hospital or Hospice Experience
  • To have or be willing to work towards PQ1 and other professional development courses

Skills and Abilities

Essential

  • Ability to assess social and psycho social needs
  • Ability to prioritise workload
  • Ability to work independently and creatively, but as part of a team
  • Skilled in verbal, written and electronic communication
  • Good interpersonal and organisational skills Sensitive approach to patients and families

Desirable

  • Knowledge of Nephrology Experience of teaching colleagues
  • Experience of multi-cultural issues

  • Awareness of equal opportunities and anti-discriminatory practice
  • Up to date knowledge of benefit system and legislation appropriate to role.
  • Flexible approach to work
  • car driver as work takes us over two counties East & West Berkshire.
