Job summary
Active Programmes is a well-developed multi-disciplinary team of nurses, physiotherapists and specialist Therapy assistants, all of whom cover both the Pulmonary and Cardiac rehabilitation. The team also comprises a small Cardiac & Respiratory Mental Health Team consisting of nurses and occupational therapists.
Main duties of the job
- To be legally responsible and accountable for all aspects of your own work including the management of patients in your care ensuring a high standard of clinical care for the patients under your management.
- To gain valid informed consent and have the ability to work within a legal framework with patients who lack capacity to consent to treatment.
- To deliver rehabilitation programmes in groups with qualified support, and in patients home setting, independently and according to local protocols and guidelines.
- To be able to take an accurate and reliable set of baseline observations including blood pressure oxygen saturations pulse and be able to identify when they are abnormal and what action is appropriate.
- To frequently assess patients to defined protocols requiring prolonged concentration to investigate and analyse the assessment to formulate individualised management and treatment plans.
- To use continuous assessment to evaluate treatment outcomes and adjust treatment plans accordingly, appropriately referring to other agencies to meet patients' needs and promote integration of services.
- To be able to facilitate patients in setting SMART goals and to review them when necessary.
