Job summary
An exciting opportunity has arisen for an enthusiastic and motivated pharmacist to join our expanding Pharmacy Homecare Team, following significant investment in the service.
This newly established post will provide clinical leadership to the Pharmacy Homecare Team and play a key role in delivering high-quality, patient-centred care through homecare services.
Working closely with clinical and pharmacy colleagues across the Trust, the post-holder will support therapeutic and biosimilar switches and contribute to a range of service improvement initiatives.
Strong communication skills, excellent time management, and the ability to work independently and collaboratively are essential.
To maintain clinical competence, the post-holder will also participate in the delivery of clinical pharmacy services during departmental slots, evenings, and weekends.
Main duties of the job
- Provide clinical leadership to the Pharmacy Homecare Team, which supports a wide range of specialist clinical services.
- Lead, develop, deliver, and evaluate pharmacy homecare medicines services, in alignment with the Royal Pharmaceutical Society (RPS)
- Professional Standards for Homecare Services.
- Lead the development, implementation, and evaluation of protocols and guidelines relevant to homecare services.
- Identify opportunities for and undertake audits, quality improvement, and service development projects to enhance patient care, safety, and experience.
- Support the education and training of pre-registration pharmacists, pharmacy staff, and other healthcare professionals on the functions and processes of pharmacy homecare.
- Contribute to the delivery of clinical pharmacy services across the department, including participation in the evenings and weekends rota.
