Job summary
This post in general surgery with an interest in oesophago-gastric surgery is for a period of twelve months commencing 2nd April 2025. FRCS or equivalent is essential. The post does not award a National Training Number. As this post is not recognised as a 'training post', full registration with the GMC with a licence to practise is required, and if the appointee does not hold right of residence in the UK sponsorship will be required. This post is suitable for an individual who has either completed their specialist training or is in the final phase of their specialist training.
Main duties of the job
The objective of this fellowship is for a combination of service delivery and advanced training in the management of oesophago-gastric disease which will include the treatment of malignant and complex benign disease. Though the fellow will be involved in providing part of the bariatric service, this fellowship is primarily intended to enhance the skills in malignant and complex-benign oesophago-gastric disease. The fellow will be provided with opportunities to enhance competencies in:
o oesophagogastric cancer resections / laparoscopic resections.
o complex laparoscopic hiatal hernia surgery and anti-reflux surgery.
o laparoscopic cardiomyotomy
o laparoscopic cholecystectomy (opportunity to be a part of the hot laparoscopic cholecystectomy service)
o upper GI endoscopies. There will be potential to get involved in ERCP, EUS and endoscopic interventions depending on the appointee's existing competencies.
o surgery for gist
o management of oesophago-gastric cancer / mdt participation
o emergency oesophago-gastric surgery
hiatal and diaphragmatic surgery
o care of the critically ill surgical patient on the hdu, itu and the ward
o weekly teaching of junior doctors and medical students

Depending upon the specific requirements of the service in conjunction with the appointees experience, skills and interest, the post is likely to involve predominantly one or two all day operating sessions, one or two half-day operating lists (including hot gallbladder sessions), and one or two half-day out-patient clinics and one or two half day endoscopy lists. There will be opportunity for audit and research and the fellow will be expected to participate actively in the mdt meeting, ward-based care of inpatients and the management of complex oesophago-gastric emergencies. There may also be an opportunity for some additional ad hoc on-call input. Post and peri-CCT fellows are also expected to provide one session every two weeks in out-patient clinic or emergency care hot-clinic for service provision.
The fellow will be provided with supervision by consultant surgeons and at the same time will be allowed to function with a degree of independence that is appropriate for the experience and capabilities of the fellow. The fellow will work within a well-established team comprising surgeons, oncologists, gastroenterologists, radiologists, pathologists and nurse practitioners.
