Job summary

We are seeking a self-motivated, reliable, and enthusiastic administrator with relevant experience to join the Transfer of Care Team. This is an established team which is integrated within Clinical Operations and encompasses all of the discharge planning services.

The successful candidate will develop interchangeable skills and may work across all aspects of Transfer of Care discharge services in an administrative capacity, this will include working closely with different pathways and services such as Care homes, Intermediate Care Services, Social Care Services, Continuing Health Care and other Community Services. They will work as part of the administration team within Transfer of Care Service facilitating effective and high-quality timely discharges across the many pathways by supporting and co-ordinating services, liaising appropriately with ward staff, relatives and service leads.

The successful candidates will be able to demonstrate good interpersonal skills in oral, and written communication, with a good ability to manage numerical data, and the ability to communicate across a range of services, agencies and professionals in a professional manner. Flexible daytime shifts are required and weekend/BH working.

Main duties of the job

The post holder will work within the Transfer of Care Service with flexibility to cover as required.The post holder will work as part of a service during the hours of 7.30 am to 6.00 pm, 365 days per year, as negotiated with the Operational Manager.Infection prevention and control is integral to this post. The post holder should at all times demonstrate effective infection control and hygiene practice.

Essential

  • GCSE C in English or above (or equivalent)
  • OCR3 (RSA III) in Word Processing or equivalent level of knowledge and ability

Experience

Essential

  • Previous appropriate experience in an office environment.
  • Basic knowledge of the NHS

Desirable

  • Experience of working in the NHS or similar organisation
  • Basic knowledge of primary and / or community healthcare

Special Skills/Aptitudes

Essential

  • Knowledge of Microsoft Office packages, databases, email and internet
  • Experienced in the use of System One.
  • Good organisational skills with the ability to prioritise a varied workload
