Job summary
Active Recovery CICS (Community Intermediate Care Service) provides care and clinical support to patients at home, to avoid hospital admission or facilitate early discharge from hospital. Active Recovery CICS is a multi-disciplinary team providing short-term intensive rehabilitation/ reablement following a holistic assessment by generic assessors. Active Recovery CICS is a city-wide service providing rehabilitation to adult patients within their own homes.
Applicants must demonstrate a strong understanding of the full Microsoft Office package in order to be successful. You will be expected to cover shifts on a Rota basis from 6am to 10pm, 365 days a year including weekends and Bank Holidays.
Main duties of the job
- Contribute to the delivery of an efficient and effective administrative service within the Department.
- Support the Active Recovery Team to deliver the service to meet the agreed objectives.
- Help to maintain live list of patients and their requirements via Administration support.
- Communicate effectively with the use of tact and diplomacy and seek to overcome barriers to effective communication. This may require the use of multiple communication methods, such as email, telephone, etc
- To utilise electronic patient records on PAS systems such as SystmOne to support the delivery of patient care.
- Support the Active Recovery Team to maintain staffing levels through booking in overtime and bank shifts as directed.
- Work collaboratively with the wider departmental team to ensure a consistent and effective Patient First.
