Job summary
We are looking for an innovative nurse case manager to join our team, working in close collaboration with colleagues from Social Care, Acute Trusts, Primary Care and care home providers to make a difference for the service user. We can offer you the time and opportunity to make a real difference to the quality of care that our patients receive in Intermediate Care.
The successful candidate will be a key member of the MDT responsible for providing person-centred, holistic assessments to facilitate discharge. Our team provides therapy and nursing input within care homes throughout the city. You will be expected to move between units as required to meet service need. Our caseload is referred from Acute Trusts often for patients that have a complex medical history including fractures, frailty, respiratory, cardiac, amputees and neurology. Patients come for short term rehab and assessments to facilitate returning home as quickly as possible.

Main duties of the job
To manage and assess patients across the intermediate care , using a management plan agreed with patient carers and colleagues, to optimise effectiveness.
To develop risk management plans with the MDT to support patients whilst in the intermediate care and on discharge.
To communicate record and report relevant information to MDT colleagues, community nursing, secondary care, and multi- agency colleagues, using the Single Assessment Process, System One and other relevant records, to ensure efficient and effective care delivery whilst in the intermediate care Unit.
To play an active role in the weekly MDT meetings to identify and progress action required to effect timely discharge ensuring desired outcomes are achieved
To work in partnership with other multi professional agencies providing support to Intermediate Care patients.
Develop knowledge, understanding and competency in professional practice in all core areas.
Continue personal and professional development as agreed with your supervisor during appraisal and supervision.
