Job summary

We are seeking a self-motivated, reliable and enthusiastic administrator with relevant experience to join the Transfer of Care Team. This is an established team which is integrated within Clinical Operations and encompasses all of the discharge planning services.

The successful applicants will develop interchangeable skills and may work across all aspects of Transfer of Care discharge services in an administrative capacity, this will include working closely with different pathways and services such as Intermediate Care Services, Social care Services, Continuing Health Care and other Community Services. They will work as part of the administration team within Transfer of Care Service facilitating effective and high quality timely discharges across the many pathways by supporting and co-ordinating services, liaising appropriately with ward staff, relatives and service leads.

Main duties of the job

  • To provide a confidential administrative support to the Transfer Of Care Team.
  • To provide cover for other administration team members as and when required.
  • Be first point of contact for the Transfer of Care Team.
  • Undertake word processing utilising advanced keyboard skills to a high standard. produce accurate documentation using Mircosoft office and email.
  • To use a range of work procedures and practice using theoretical knowledge to maintain effective filing and record keeping systems, including the indexing of files and retrieval of information using appropriate software programmes.
  • Contribute to the effective maintenance of office systems and procedures, carrying out administrative duties as required, including photocopying, faxes and mail-outs.
  • Provide and receive routine, complex or sensitive information and on occasion exchange information with patients, carers or relatives using appropriate tact.
  • May be required to exchange complicated information within the team.
