Main duties of the job

We are seeking an experienced and proactive Nurse to join our dynamic, nurse-led anticoagulation service. This role offers a unique opportunity to provide expert care across all stages of anticoagulant treatment--from initiation and patient education to peri-operative management and treatment cessation.

This role involves working in collaboration with health and social care colleagues within the Trust and external primary and acute care providers to develop patient-centred pathways of care for smooth and timely referral, minimising delays and supporting shared care.

You will work closely with Consultants and fellow Clinical Nurse Specialist to deliver high-quality, patient-centred care within a supportive multidisciplinary team. This is a key clinical role requiring strong communication skills, clinical judgement, and the ability to support patients and families during potentially anxious and complex periods of treatment.

Job responsibilities

Responsibilities include:

Comprehensive reviews for new patients commencing anticoagulant therapy.

Actively involved in the management of the UHS Nurse Led Anticoagulation and Thrombosis.

Pre-operative bridging service of stable and complex cases.

Provide advice to staff trust wide and primary care on anticoagulation issues as required.

Delivery of education and clinical updates on venous thromboembolism (VTE) across the trust to all staff as required.

Support the Trusts VTE prevention strategy, including risk assessments and thromboprophylaxis in line with national recommendations.

Experience and skills needed include:

Registered general nurse

Excellent communication and interpersonal skills allowing you to support patients and their families effectively during challenging times.

Awareness of owns education needs and wiliness for self-development to complete Anti-coagulation and thrombosis competencies and work towards becoming an Independent Nurse Prescriber.

  • Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience or portfolioMultidisciplinary team working.
  • Significant post registration experience / qualification / training, including experience and knowledge within relevant speciality
  • Completion of all Band 5 (preceptorship) competencies
  • Higher Education Institute approved course in supporting learners in practice or evidence assessed to be of equivalent experience, demonstrating support to a range of learners in practice.
