Job summary

We are seeking a motivated, enthusiastic and positive clinician to join the complex discharge team as a complex discharge team lead. This post will cover 10 months of maternity leave and would be an excellent secondment opportunity if you are looking for new challenge and an exciting change.We would welcome applications from Registered AHPs, and Nurses with a proven track record of leadership and a passion for patient centred care. You will receive support to develop your leadership expertise, operational planning for the service and supervision of others alongside clinical reasoning for patients.

The successful applicant will form a part of a strong team made up of Band 8 ops leads, Band 7 Team leads, Band 6 practitioners, band 4 advanced discharge officers, and band 3 discharge officers. The post supports clinical specialties across the trust, links with multiple internal/external teams & draws upon your clinical skills to effectively support our most complex patients.

Your Responsibilities What youll do

The post holder is responsible for managing the teams undertaking the assessment and discharge planning of patients within the defined clinical area, working under direction of their Complex Discharge Operational Lead. In addition, act as a case manager using their clinical knowledge and experience to support the day to day operational functions of the teams, completing their own assessments and interventions where indicated, ensuring referrals are made in a timely way to support flow across the organisation. Understanding each person within the patient assists external providers in meeting the patients needs, so advanced skills in communication are essential.

What were looking for

You will be expected to be a confident communicator with excellent organisational skills. You will be required to liaise with family members, patients, and members of the Multi-Disciplinary Team on a regular basis and be competent with IT systems that facilitate this. You will need to demonstrate an ability to work in a busy and often pressurised environment, be able to provide highly specialised clinical discharge planning advice both within the Trust and externally, to other team members, and to health and social care professionals. A passion to get it right first time for our patients is what drives us. In return we can offer you the opportunity to develop your leadership and managerial skills and gain a wider knowledge with regards to Health and Social care systems and processes. You will also form an integral part of the Trust wide training strategy for complex discharge.

Essential

  • First level Registered Nurse, Physiotherapist or Occupational Therapist
  • Professional and clinical knowledge around complex discharge issues, or a willingness to develop such knowledge.

Desirable

  • Recognised teaching/mentorship qualification or equivalent experience
