Job summary
We have an exciting opportunity to recruit an enthusiastic Band 8a Lead Advanced Clinical Practitioner (ACP) to the Older Persons Decision Unit (OPDU) for 1 year.
About the team
The ODPU sits within the Same Day Emergency Care (SDEC) service at the front door of the hospital and is pivotal in supporting admission avoidance and reducing length of stay for over 80s presenting with a medical condition. It is a forward thinking integrated team made up of professionals from both UHS and Solent NHS Trusts to utilise, and further develop, seamless pathways between primary and secondary care for people over 80.
The team consists of a Consultant Geriatrician, a Geriatric Specialist Registrar, an SDEC junior doctor, SDEC nurses and 8 ACPs. The ACPs are an integral part of the team ensuring appropriate patients are assessed in a timely way with early senior decision making. They also play an important part in following up high risk patients to prevent readmission to hospital.
