Job summary

An exciting opportunity has arisen for a dynamic occupational therapist to join the Community Therapy team in Bassetlaw . The team provides short-term rehabilitation to older people in their own homes to avoid admission to hospital and support discharges from hospital ( D2A).

This is a challenging but rewarding role with a strong multidisciplinary approach within the team.

The post covers Bassetlaw area and is based at Retford PCC working shifts between 08:00 and 20:00 seven days a week to meet the diverse needs of this patient group.

In addition to support from your team leader and the MDT, OT specific support is provided by monthly peer supervision and forums which offer support and CPD.

This is a 9 months maternity leave cover for 30 hours per week.

Main duties of the job

To provide an occupational therapy service for older people with complex health needs who are referred to the service.

To ensure a high-quality Occupational Therapy Service is provided through multi-disciplinary, multi-agency working. To carry out specialist assessment, goal setting, implementation of treatment programmes and evaluation.

To establish the needs of individuals referred to the service and plan care with colleagues from health, social services and the voluntary sector.

To represent and promote the specialist role of Occupational Therapy within the Community Team including involvement with service planning and development.

To provide specialist advice, rehabilitation and equipment to the population of Bassetlaw who access the service
