Job summary

Are you a motivated District Nurse looking for change? We are seeking our next District Nurse to join the Newark District Nursing Team.

The Adult Community Service is an integrated healthcare team providing long- term conditions management, rehabilitation and community nursing services within the Newark town and surrounding villages . We are an enthusiastic team with a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in both a home and clinic setting.

Main duties of the job

Your role will be to lead a team of staff and case manage the provision of care within an individual's home or residential home, including palliative care and long term conditions management. You will work autonomously using your own initiative to deliver holistic care to adults with physical health needs.

A key aim of our service is reduce unplanned hospital admissions and facilitate discharges from the acute care environment. The team members include physiotherapists, occupational therapists, Community nurses, community matrons, specialist nurses and support workers. We also work closely with GP's and the voluntary sector
