Community rehabilitation in a physical healthcare setting is on the cutting edge of current healthcare provision. Discharge to assess and urgent care reablement models require patients with complex needs to be assessed at home to optimise function. Health promotion and prevention of or managing disease progression is also key to keeping our population healthy.
This post gives you opportunity to be involved in these areas of healthcare provision. Community based working in a fully supportive MDT.
This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.
Main duties of the job
We need a Band 5 Occupational Therapist who is motivated and able to work in a recovery focussed way to join our Community Therapy team based in Rushcliffe. Our integrated Healthcare Team provides goal focused, needs led treatment to those with complex long term conditions. In addition, your role will provide experience in admissions, avoidance, falls prevention and the discharge to assess pathway. We strive to provide an evidence based service and to ensure that all service users are valued, listened to and receive high quality personalised care. The team members include physiotherapists, occupational therapists, community nurses, matrons, specialist nurses and support workers, also working closely with GPs and voluntary sector.
You will have a degree/diploma in Occupational Therapy and be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given.
The service currently runs from 8am - 8pm, 7 days a week, therefore some weekend and bank holiday working may be required.
A full UK driving licence is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
