Job summary

We are a new team looking for an enthusiastic team player to join us as a pathway coordinator.

The post holder will provide coordinated care for patients with Severe Multiple Disadvantage (SMD) . The post holder will be responsible for providing a high quality communication and liaison service across a wide range of services determining and signposting service users and staff to appropriate services that support integrated, multidisciplinary working.

This requires the receiving and processing of referrals from a range of agencies and effective communication across professional organisational boundaries, ensuring equality of access across full range of services in a timely manner in lines with agreed protocols.

This post is a critical part of the newly formed SMD team.

Please note that this post does not meet the pay level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

Main duties of the job

To act as the main focal point for prioritising the communications within the SMD team.

To identify and provide the relevant information to the team to ensure they are fully aware of the patients needs.

To work in partnership with the team to ensure agreed pathways are followed, communicating with health and social care colleagues, statuary and voluntary organisations.

Build and maintain a wide network of relationships across a variety of organisations as multiagency working is vital to the success of this team and to ensure the patients receive the appropriate support and care

To respect confidentiality for work undertaken and can use their initiative, work with minimum supervision to deadlines and maintain a flexible approach.

  • NVQ L4 in a relevant subject or recent demonstrable experience

  • IT qualification

  • Experience of working in a busy administrative role and/or customer care setting
  • Understanding of the principles of Confidentiality, Equal Opportunities and Equality and Diversity
