We are pleased to advertise the exciting role of team manager for the Patient transfer of care team , which will involve managing , patient transfer of care team, street triage, in reach workers and out of area in reach

The role involves:

To be responsible for the operational management of The Trust's Patient Transfer of Care Team. The team is responsible for managing the admissions and patient flow to The Trust inpatient and subcontracted beds. Managerial responsibility extends to the "In reach" workers based at Highbury Hospital and Sherwood Oaks based in Mansfield.

The post holder will also hold managerial responsibility for the Street Triage Team ( in partnership with Nottinghamshire Police) based at Police Headquarters at Sherwood Lodge in Nottinghamshire.

The post holder will support and promotes the delivery of high-quality health service to a diverse group of patients, their families, and carers, in accordance with quality practice and procedures.

To provide continuing leadership and development responsibilities ensuring the effective delivery of clinical/evidence-based practice by the team in line with the clinical strategy, service specifications and operational & financial parameters.

Main duties of the job

AMHhas developed a specialised Patient Transfer of Care Team to manage admissions and transfers to our Trust Inpatient acute inpatient beds alongside the Trusts Subcontracted beds.

The Team Leader will be responsible for the management and leadership of clinical and administrative staff in the Patient Transfer of Care Team, ensuring that patients receive clinically effective services and that a suitable inpatient bed is sourced at the earliest opportunity and closest to home once a patient has been "gatekept" and prioritised for an acute or PICU admission and that agreed performance targets are met.

The Team Leader will work closely with other relevant stakeholders to ensure strong leadership to ensure timely admissions and the effective use of inpatient beds and sub-contracted beds.

The management of team budgets.

There is an expectation of participation with the on-call rota providing management and leadership support out of usual hours to AMH services.

To ensure the development of services in line with the business plans, particularly the implementation of the Trust aims and objectives, developed in line with National Targets.

To understand and appreciate organisational structures, business strategy and the work of other departments for the development of the service.
