Job summary

As part of the wider Nottinghamshire multi-disciplinary Intellectual Disability (IDD) Community Team working specifically within the IDD Single Point of Access Team screening referrals and establishing people's eligibility to receive support from community IDD services.

This role requires an individual with the skills, expertise and sound knowledge in working with individuals with an Intellectual Disability. The individual will be able to work as part of a team as well as autonomously and being able to work alongside the Qualified IDD SPA Clinician to prioritise referrals coming into the service according to their level of need.

You will be experienced with sourcing information from people with an IDD, carers and wider MDT via the telephone as well as identifying and responding to any safeguarding concerns.

Maintains a professional attitude as a representative and first point of contact for referrers to IDD Community services.

Main duties of the job

A new and exciting opportunity has arisen for a band 3 Clinical Assistant to work within our IDD Single Point of Access, screening referrals and establishing people's eligibility to receive support from community IDD services.

We are looking for someone with enthusiasm and energy who wants to make a difference to people's lives; is creative and innovative and highly motivated. You must have assessment and care planning skills and experience in multi-disciplinary working and supporting people with a learning disability and Autism.
