Job summary
The Community Diabetes Service provides support, management and complex case advice for adult service users with diabetes, within the Sherwood area . The team acts as a source of expert advice and support to GP's, Practice Nurses, Community Matrons, District Nurses and Care Homes. The team also has a specialist education role within primary care facilitating education and training for all staff and providing structured education to adults with Type 2 diabetes.
You will deliver diabetes advice, care and support to patients / health and social care professionals. You will act as an educator, expert clinician and source of support to other primary care staff and staff working in Residential and Nursing Homes. You will need to work autonomously within a multidisciplinary environment. You will be flexible, reliable, and approachable.
Main duties of the job
An exciting 30hr opportunity has arisen for an experienced nurse to work within the established Community Diabetes Nurse team within the Mansfield and Ashfield Locality. If you are an independent prescriber, with a diabetes nursing qualification we would love to hear from you.
The successful candidate will offer and provide joint clinics with GP's and Practice Nurses. The successful candidate will be expected to demonstrate excellent communication and interpersonal skills with the ability to manage and lead change within the community
The successful candidate will have a post registration diabetes qualification or relevant experience and be a non-medical prescriber. You will have experience in providing education and training to groups of patients and staff.
