Job summary

These posts are subject to National NHS T&Cs, paid enhancements for unsocial hours working during nights, weekends and bank holidays and we offer a Golden Hello payment of £2,750 to new starters with the Trust in this role (payable in instalments over a 2 year period)

An exciting opportunity has arisen for an experienced Practice Nurse to work within our Substance Misuse Team at HMP Lowdham Grange, providing care to offenders that is equivalent to that in the community

The substance misuse team at HMP Lowdham Grange has recently been restructured to introduce changes to the way we work with our Service Users. This is an exciting time where we have the opportunity to develop a service with excellent provision and positive outcomes.

The Substance Misuse Team is made up of Nurses, Pharmacy technicians, a GP with Special Interest/NMP and Psychosocial Staff to provide the whole package of care for those individuals receiving treatment and support. The team provide Psychosocial, Psychological and pharmacotherapy interventions to ensure the opportunities for sustained recovery are explored and maximised not only whilst in custody but when our Service Users return to the community.

Prison Healthcare - ever wondered what it was?

We aren'ta hospital in the Prison.

We don'thave in-patient beds.

We dooperate as a Primary care centre, with some urgent treatment provision.

We dohave general nurses, mental health nurses, substance misuse practitioners, an advance nurse practioner, GPs, physio, dentist, optician, podiatrist , paramedics and psychiatrist and so on....... over 40 staff infact !!

Andwe dohave an exciting opportunity to recruit additional Band 6 Senior Nurses to work within our Integrated Mental Health Team at HMP Lowdham Grange, providing a level of care to over 800 men that is at the least equivalent to what they would expect to receive in the community.

Job responsibilities

To carry a complex case-load. you will hold responsibility and accountability for comprehensive assessment, planning, formulation and evaluating patient care and positive outcomes for those referred to our service. You will ensure Care Plans are individual to the patients' and SMART.

To plan programmes of clinical and psychosocial interventions using evidences based practice, utilising relevant clinical tools both clinical, psychosocialand psychological.

To provide information and advice about all substances (including drugs, alcohol, psychoactive substances and prescribed medications) on which individuals can become dependentwhich impacts on daily living.

To provide information and advice on Blood Bourne Viruses and safer sex.

To facilitate group-work relevant to substance misuse and offending behaviour. to provide smoking cessation advice support and interventions in line with National Guidance and locally agreed targets.

To actively liaise and develop links with services within the prison setting and in the community to establish effective throughcare and after care pathways. Ensure those leaving our care have a safe discharge.

To assess and respond to clients/patients at risk of suicide or self-harm using appropriate interventions, prison service ACCT procedure, drug screening, interpreting the results and refer to appropriate part of our integrated service.

To hold a radio as first or second response in attending to those thought to be under the influence of unknown substances. Ability to complete physical observations using NEWS2 and COWS, escalating appropriately based on your clinical findings. To attend training sessions including Trust mandatory training.

Responsibility for the safe storage, handling and administration of medications including controlled drugs.

To undertake any other duties that would be a reasonable expectation of the role.
