Main duties of the job
We are currently looking for enthusiastic, reliable and adaptable Registered Mental Health Nurses & Learning Disability Nurses to join the Trust Bank providing support to Crisis Resolution and Home Treatment Team. This role requires a clinician with specialist skills to assess patient's needs, analyse and develop treatment plans within a multidisciplinary framework.

The team provides a service 24 hours, 7 days per week, with a wide range of shift patterns.
The role of Band 6 Crisis Care Practitioner consists of answering 111 telephone calls, assisting with the query/concerns that arise, sign posting to relevant authorities, problem solving, shift lead and you will also be required to carry out assessments. You will work with a well-established team of Psychiatrists, Nurses and Support Workers providing rapid response for adults who are experiencing an acute psychiatric crisis. The service 'gate keeps' the inpatient beds and provides care in the least restrictive environment by offering an alternative to hospital admission through intensive home treatment.
Due to the nature of the work, a full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

You must be an RMN or OT, hold current registration and have considerable experience of working with people with mental health problems in an acute health care/ community setting.

You must have the ability to work as part of a multidisciplinary team, be reliable, motivated. You will need to demonstrate high functioning in risk management, communication skills with exceptional personal qualities to deliver effective care. The post holder will be required to assess the needs of individuals when they are acutely unwell, develop and continually review treatment plans that enable the individual to manage their period of crisis effectively.
You will be required to attend an interview and complete competencies in order for you to be successful within the role of a crisis care practitioner.
