Community Psychiatric Nurse: County Dementia Outreach ServiceMental Health Services for Older People Newark & Sherwood Integrated Community Mental Health Services is an established service offering assessment and treatment for older people with mental health difficulties and for people of any age with organic diagnosis within the Newark & Sherwood locality. The County Dementia Outreach Service aims to improve the quality of care for individuals with dementia living in dementia registered residential and nursing care homes in the Newark & Sherwood locality.
Main duties of the job
We are looking for a committed and experienced Band 6 Community Psychiatric Nurse to help deliver the service inNewark and Sherwoodas part of the locality community mental health team for older people.
You will be based within the Multi-Disciplinary Community Mental Health Team whilst also being a part of the wider outreach service which is managed by a clinical specialist. A key aspect of the Community Psychiatric Nurse role will be advising and supporting care home staff to improve their skills and confidence to manage behaviours and promote person centred care planning. Specialist knowledge of the nursing issues associated with dementia in a residential setting and a high level of communication skills are crucial.
The County Dementia Outreach service covers the County of Nottinghamshire and operates within a "virtual team" service model with clinicians being based within locality based community mental health teams.
