This is an exciting opportunity for an exceptional individual to join our Community Adult Speech & Language Therapy Team. The post consists of:
3 days a week working within a specialist team in Bassetlaw that provides 2 services, based at Retford Primary Care Centre (PCC):
The Stroke Early Supported Discharge ( ESD ) Service. The multidisciplinary team provides an early, intensive rehabilitation service for stroke patients, with the aim of enabling patients to leave hospital more quickly after their stroke, and to return to their own homes. There is close working with other MDT members.
The Bassetlaw Long-term Neurological Service aims to provide a comprehensive nursing, assessment and rehabilitation service to support patients with clinically diagnosed long term neurological conditions. The service will maximise potential enabling patients to live within their community.
The SLTs within these services, also provide assessment and management to people with dysphagia secondary to dementia, and other medical conditions.
2 days a week working in the Mid Notts Long Term Conditions ( LTC) team based from Mansfield Community Hospital ( MCH):
The team works with people who have a long term neurological conditions providing treatment and management from early diagnosis through to the later stages of care ensuring the client's needs are met. The team provide Lee Silverman Voice Treatment LOUD therapy in groups as well as other evidence based treatments.
Main duties of the job
The role requires clinical and professional expertise in order that those with speech, language and communication needs and/or dysphagia needs benefit from a well-coordinated pro-active, specialist, efficient and effective speech and language therapy service.
The post holder is responsible for specialist SLT assessment, contributing to differential diagnosis, individual and group therapy and management, training and advice as appropriate.
The post holder will have a delegated leadership and responsibility for the SLT needs within the team. They will identify and advise around needs and trends and together with their line manager they will develop and provide training as appropriate.
