This is an exciting opportunity for an exceptional individual to join our Community Adult Speech & Language Therapy Team. The post consists of:
3 days a week working in Bassetlaw within 2 services, based at Retford Primary Care Centre (PCC):
Community Stroke & Neuro Rehabilitation Team: Working within a multidisciplinary team that provides intensive rehabilitation for stroke patients and a comprehensive nursing, assessment and rehabilitation service to support patients with clinically diagnosed long term neurological conditions. The service aims to maximise potential enabling patients to live within their community. There is close working with other MDT members.
Bassetlaw Unidisciplinary Speech & Language Therapy Serviceaims to provide assessment and management to people with dysphagia and communication needs secondary to dementia, and other medical conditions.
2 days a week working in the Mid Notts Long Term Conditions ( LTC) team based from Mansfield Community Hospital ( MCH):
This specialist outpatient team works with people who have long term neurological conditions providing assessment, treatment and management from early diagnosis through to palliative care. We work as a uni-disciplinary service however, have excellent links with specialist teams across the county including the Parkinson's Specialist team, Neuro Rehab therapists and our regional Electronic Assistive Technology Service.
We provide patient focused care consistent with national initiatives for people living with long term conditions. We strive to offer evidence based therapy such as Lee Silverman Voice treatment and embrace new treatment initiatives eg EMST. We have a robust programme of treatment groups operating across the county
We work in clinic bases, patients own homes and care settings dependant on client need. We offer individual and group therapy options and have the support of a Speech and Language Therapy Assistant.
Main duties of the job
The role requires clinical and professional expertise in order that those with speech, language and communication needs and/or dysphagia needs benefit from a well-coordinated pro-active, specialist, efficient and effective speech and language therapy service.
The post holder is responsible for specialist SLT assessment, contributing to differential diagnosis, individual and group therapy and management, training and advice as appropriate.
The post holder will have a delegated leadership and responsibility for the SLT needs within the team. They will identify and advise around needs and trends and together with their line manager they will develop and provide training as appropriate.
