Main duties of the job
Our integrated teams of Nursing and Allied Health Professionals deliver care to people who present with significant physical health care needs in various settings including clinic based, community inpatient, hospices to care in the patients own home, including care homes, enabling them to be independent, self-caring and live their optimum lives, reducing hospital admissions.
Services range from community nursing, including specialist practitioners, to public health and health promotion such as school nursing and health visiting, with many specialist community services designed to support care pathways such as Physiotherapy, OT, Speech and Language, Dietetics and Podiatry.
Supporting the efficient running of these services are administrative and managerial roles. All roles, at all levels, are vital in transforming community health services, ensuring high quality care is provided both now and into the future based around the needs of our patients - together we all make a difference.

We are always looking for great Health Care Assistants to join our community nursing teams who on the road delivering care into peoples homes. We have opportunities to work in Mid Nottinghamshire specifically Mansfield, Ashfield, Newark & Sherwood, 7 days a weekincluding Bank Holidays.
Personal and work commitments is sometimes difficult to manage, and we will support people to achieve the right work life balance through flexible working in a variety of full and part time roles which aim to maintain the health and wellbeing of our workforce.
The role requires day-to-day delivery of patient care in our Integrated Community Teams led by District Nurses, Allied Health Professionals and Social Workers . together you all make a difference.
You will work within the Mid Nottinghamshire Community Urgent Response and Rehabilitation Team, under the direction of a registered professional, with patients who require support to carry out activities of daily living in order to maintain them within their own home environment.

Demonstrate high levels of care and compassion to assess and deliver care to patients who present with significant physical health care needs within the places they call home.
Enjoy working collaboratively within a multidisciplinary framework, analyse and develop treatment plans whilst providing compassionate, realistic and timely support to a diverse patient group and their families
