An exciting opportunity has arisen for a District Nurse to lead three dynamic teams within the north and south localities of Rushcliffe. The Adult Community Service is an integrated healthcare team providing long- term conditions management, rehabilitation and community nursing services within the Rushcliffe area. We are an enthusiastic team with a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in both a home and clinic setting.
Your role will be to lead a team of staff and case manage the provision of care within an individual's home or residential home, including palliative care and long term conditions management. You will work autonomously using your own initiative to deliver holistic care to adults with physical health needs.
