Are you a Mental Health Nurse RMN looking for a new challenge in Older Persons Mental Health Services?

The Taunton & West Somerset Older Persons Integrated Community Mental Health Team (OPMH) are inviting Mental Health Nurses (RMN) to apply for an exciting opportunity to work within the CMHT. We are looking for an exceptional RMN that has both the experience and passion to work with older people with severe and enduring mental health difficulties.

Main duties of the job

The post holder will be a member of the multi disciplinary Older Person's Mental Health Service. They will work in the community mental health team and dementia assessment service, offering specific input tailored to the needs of older people who experience varying degrees of cognitive and mental health problems, to improve their quality of life, maintain their existing skills and provide carer support.

Working with all cases, a warm, sensitive and stimulating environment needs to be created and you will need to demonstrate the ability to work as part of a team. Flexibility is a key quality and you must be able to respond to clients in an appropriate manner. You will have good communication skills relating with clients and the carers, and be able to demonstrate an ability to understand their needs and where possible, be able to help the carers. You will be required to visit clients in their homes, so it is essential you have a driving licence and access to a vehicle. You will also have some specific administration responsibilities therefore IT skills would be an advantage.

Responsibilities with Patient Care

*Undertaking care coordination responsibilities with a defined caseload focusing on work with people experiencing severe and enduring mental health difficulties and other complex needs and difficulties in the community.

*To undertake the assessment of patients referred to the Older Persons' Mental Health Service using a recovery approach and strengths model. Undertake the assessment of risk and nursing needs and contribute to the multi-disciplinary team care plan, memory assessments, under the development and implementation of individual care plans for the client.

*To actively participate in Primary Care Network Multi-Disciplinary Team meetings in Taunton and West Somerset discussing PCN patients, including care home residents

*To provide high quality patient care, working in a flexible way across the community, and day hospital, as required, focussing on the need to support patients and their families/carers.

*To fully comply with the Trust and Local Authority Policies relating to the ICPA/Care Management and act as a key-worker for patients.

*Use interpersonal skills to build a therapeutic relationship with patients.

*To provide support and interventions to patients in their own homes (including care homes) to enable them to remain in the community during crisis periods.

*Providing and receiving highly complex and sensitive information in relation to Safeguarding and Child Protection work and taking appropriate action. This includes identifying and appropriately reporting concerns to Children's Social Care while using communication skills in order to maintain a professional working relationship with parents and significant others in often hostile and highly emotive situations.
